Public hospitals: End of fee-for-service pricing in hospitals, better pay for carers
Direction of change: increase, compared with current law.
The lever. Resources for public hospitals: budget, beds, staff numbers and pay of health workers, activity-based funding model. Direction rule: ↑ more resources for public hospitals.

Current law, sources and limits of the cost schedule
Current law. ONDAM 2026: the « établissements de santé » (health establishments) sub-target funds public and private establishments, at €111.8bn (PLFSS 2026 base). The PLFSS planned +2.4%; in November 2025 the Government announced an €850m increase bringing growth to +3.2% (explanatory statement of amendment no. 2695, which fell at first reading; the final text of the LFSS 2026, which sets the sub-target as voted, has not been reviewed).
Horizon. full year 2032, €bn/year
Limits. 1% of €111.8bn = €1.12bn today. With about 3% growth per year until 2032 (assumption), the base approaches €137bn, hence €1.3bn to €1.4bn. Commitments of “+X care staff posts” require a cost per FTE in the hospital civil service, not sourced here. The sub-objective includes the for-profit private sector: a position that targets public hospitals specifies this.
How to read these amounts. These are static costs and revenues, with no feedback effect, over a full year: nothing on growth, employment or the revenue that economic activity would gain or lose. A higher net cost does not on its own make a worse policy, nor net savings a better one: each programme rests on an economic model (demand-led stimulus, lower compulsory levies, consolidation of the public accounts…) that the cost schedule does not settle. GDP itself measures a volume of activity, not well-being or the usefulness of what is produced. Economic models and feedback effects.
