Medical deductibles: Abolish medical deductibles and flat-rate charges, aim for 100% social security

Direction of change: abolition of a scheme, compared with current law.

The lever. Sums left for the patient to pay on medicines, consultations and transport, not reimbursed by national health insurance (Assurance maladie). Direction rule: ↑ higher deductibles.

« Nous proposons exactement le chemin inverse : supprimer les franchises médicales et les participations forfaitaires et aller vers le remboursement à 100 % par la Sécurité sociale de tous les soins prescrits, sans reste à charge. »1

Translation by the editorsWe propose exactly the opposite path: abolish medical deductibles and flat-rate charges and move towards 100% reimbursement by social security of all prescribed care, with no out-of-pocket cost.

Costing

Announced by the candidate

not announced

The cited source does not cost this measure.

Published costings

none

No external costing of this position recorded to date.

Our calculation

−€2.1bn

per year for the public accounts, revenue cut, range −€2.3bn to −€2.1bn.

The calculation

  • Total abolition of medical deductibles and flat-rate contributions: Option costed by the source: a cost of €2.1bn (2.1 to 2.3) per year.

Partial costing. The cost schedule option matches the text, which names the medical deductibles (franchises) and flat-rate contributions. 100% reimbursement of all prescribed care, targeted without a timetable (« aller vers » [“move towards”]), is not costed: the grid has no lever for out-of-pocket costs other than deductibles. The end of extra fees (dépassements d'honoraires), in the following sentence, is recorded under its own lever.

Current law, sources and limits of the cost schedule

Current law. Charges (franchises) of €1 per box of medicine or paramedical procedure and €4 per transport; flat contribution of €2 per consultation or procedure (doubled in March and May 2024). Annual caps raised from €50 to €70 each, i.e. €140 combined instead of €100 (decree no. 2026-858 of 11 September 2026, from 1 October 2026). The €200 plan was abandoned.

Horizon. full year 2032, €bn/year

Limits. Abolition: central value and lower bound, €1.3bn (stable yield from 2009 to 2020, general scheme, REPSS) + €0.8bn (2024 doubling, government estimate) ≈ €2.1bn; upper bound, €1.5bn (2021 yield, REPSS) + 0.8 ≈ €2.3bn (editors' calculations). The values of €2.4bn and €2.8bn used until 18 October 2026 were not based on any cited source: withdrawn. The 2026 raising of the ceilings (yield not published) is not counted: it would slightly increase the cost of an abolition. A media outlet (Le Tribunal du Net) puts forward €2.5bn in 2025 without a checked primary source. The revenue figures are amounts due: €1.5bn of receivables remained to be collected at the end of 2024 (Cour des comptes, via iFRAP). Notable discrepancy: the PLFSS 2026 costed the initial plan (doubling of amounts and ceilings) at €2.3bn, while Caducée speaks of about €750m for the move to €200 alone.

How to read these amounts. These are static costs and revenues, with no feedback effect, over a full year: nothing on growth, employment or the revenue that economic activity would gain or lose. A higher net cost does not on its own make a worse policy, nor net savings a better one: each programme rests on an economic model (demand-led stimulus, lower compulsory levies, consolidation of the public accounts…) that the cost schedule does not settle. GDP itself measures a volume of activity, not well-being or the usefulness of what is produced. Economic models and feedback effects.
