Where doctors set up practice: Freedom to choose practice location, contracting differentiated by need

Direction of change: change of another kind, or uncertain direction, compared with current law.

The lever. Rules on where doctors set up practice across the country: freedom, incentives, or regulation in areas already well served. Direction rule: ↑ more regulation of where doctors set up practice.

« Garantir la liberté d’installation des médecins et instaurer un conventionnement différencié selon les besoins définis localement par les élus et communautés soignantes, afin d’orienter l’offre là où elle manque le plus. »1

Translation by the editorsGuarantee doctors’ freedom to choose where to set up practice and introduce differentiated contracting according to needs defined locally by elected officials and healthcare communities, in order to steer provision to where it is most lacking.

Current law, sources and limits of the cost schedule

Current law. Doctors are free to set up practice where they wish. Zoning and incentive payments by the ARS (regional health agencies). Selective agreements with health insurance (conventionnement sélectif) apply to other professions (nurses, physiotherapists). The cross-party Garot private member's bill on regulation was adopted at first reading in the National Assembly in 2025 (parliamentary shuttle not checked).

Horizon. Not applicable.

Limits. Regulating where doctors set up practice has almost no budgetary cost. Aid for setting up practice and the funding of the 4th year of general practice training can be costed, but are not sourced here.

How to read these amounts. These are static costs and revenues, with no feedback effect, over a full year: nothing on growth, employment or the revenue that economic activity would gain or lose. A higher net cost does not on its own make a worse policy, nor net savings a better one: each programme rests on an economic model (demand-led stimulus, lower compulsory levies, consolidation of the public accounts…) that the cost schedule does not settle. GDP itself measures a volume of activity, not well-being or the usefulness of what is produced. Economic models and feedback effects.
